Minimum Qualifications Preferred:

One year of experience in case management, protective services, or a related field, or an equivalent combination of training and experience.
Required:

A minimum of two years of professional experience in a human services program providing social services to clients AND a bachelor's degree in social or behavioral sciences, public health, or a related field.
OR
-
An equivalent combination of education and professional experience providing social services to clients in a human services setting.

Job Duties and Responsibilities -
Respond promptly and professionally, either verbally or in writing, to inquiries from internal departments, external agencies, clients, families, and other stakeholders.

Provide input on policies and procedures and may participate in incident reviews, closing reports, and investigations.

Assist in tracking and reviewing case information, compiling data, and preparing reports for supervisors and leadership as needed.

Maintain a caseload of APS cases that require long-term funding or intensive casework, particularly when vendor involvement has reduced but not eliminated risk.
Case Investigation Responsibilities:

Review active APS cases and conduct thorough investigations, including outreach to collateral contacts such as family, alleged perpetrators, healthcare providers, banks, neighbors, and care agencies.

Adhere to all APS legislation, regulations, and policies, ensuring compliance with required legal timelines.

Assess unmet needs or risks, and when applicable, refer cases back to the APS vendor for continued service planning.

Collaborate with internal and external program offices to identify and remove service barriers in complex cases.

Close cases with no unmet needs or ongoing risk in accordance with the APS database manual and procedures.

Utilize agency-based medical professionals for assessments when needed.

Accurately document all investigative actions and outcomes in the Pennsylvania Department of Aging's standardized documentation system.

Perform additional related duties as assigned.
Essential Functions -

Successfully complete all required APS training programs as directed by the Pennsylvania Department of Human Services.
