Basic Job Functions:
Chart Pickup From Various Hospital Units on a Rotating Schedule
Chart Assembly with Scanning Preparation
Batch Scanning Records into EMR System
Indexing Documents to the Correct Patient, Visit and Document Type

Qualifications:

Strong computer skills with a focus in Microsoft applications. Must possess the ability to prioritize work and meet specific deadlines with strict time constraints. Ability to successfully multitask, with strong interpersonal skills. Ability to interact with all members of the organization, including but not limited to, medical staff. Attention to detail and analytical skills are a must. Experience with medical record assembly and/or terminology is preferred.

Education Requirements:

High School Diploma or Equivalent

Major Tasks, Duties and Responsibilities:

Orient self with hospital unit names, locations and containers.

Comfort with speaking to clinical staff to confirm all charts are received when rounding.

Determine appropriate admission and discharge in the EMR system for accurate filing of documentation.

Group documents together and ensure quality guidelines are followed to prepare for batch scanning.

Ability to batch scan in both or current EMR and Legacy applications.

Determines the readiness of the batch for the Indexing process.

Locates the electronic record for validation of the paper batch.

Reviews each image in the batch and verifies the correct patient name and ECD number, proper document type and image quality including legibility, orientation, etc.

Rearranges out of order images within an electronic document.

Replaces electronic images and documents that have unacceptable image quality or have been updated.

Communicate system or user concerns to manager.
Achieves and maintains established productivity standards.
Maintains confidentiality of patient information according to established HIPAA guidelines.

Physical effort:

Heavy workload with standardized timeframes for completion.

Must be able to occasionally walk for long periods of time and stoop, kneel, bend, crouch and lift.

Prolonged PC work with long periods of sitting.

Ability to push/pull up to 50 pounds.
