We are currently seeking a Referral Coordinator to join a small, dedicated team within a mission-driven healthcare organization. This non-patient-facing role is perfect for someone who thrives in a supportive, detail-oriented environment and enjoys working behind the scenes to ensure patients receive the care they need.

Key Responsibilities

Direct Patient Services
  • Ensure accurate and complete referral/admission and patient tracking information by initiating and following up on all internal and external referrals on behalf of the Health Center's patients.
  • Serve as an Ombudsman for the Health Center's patients and MCOs/HMOs regarding access to non-primary care services, including coordination of the appeals process for denied services.
Administrative Duties (50%)
  • Keep detailed, accurate, and complete records of referral activities to ensure appropriate visit frequencies as required for each payer and discipline.
  • Review utilization and referral reports from all payors, identify trends/patterns, and report findings to management.
  • Distribute referral reports to appropriate stakeholders.
  • Maintain and make available to providers documented referral protocols, policies, and procedures for all contracted MCOs and HMOs.
  • Participate in outreach and education activities to attract new members and retain current membership.
  • Perform other related responsibilities as assigned.
Team Structure
  • You'll work as part of a tight-knit team of three: 2 Referral Coordinators and 1 Manager
Must-Have Qualifications
  • 1+ year of experience handling healthcare referrals
Nice-to-Have Skills
  • Bilingual (Spanish preferred)
  • Experience using EPIC
  • Familiarity with FQHC (Federally Qualified Health Center) settings
