Founded in 1985 as a subsidiary of the organization then known as Church Home Foundation, Conexio's original mission was to help older adults with lifelong histories of psychiatric hospitalization to transition into the community. POSITION STATEMENT: A Quality Specialist is responsible for ensuring that services meet the required regulations per Department of Health and Social Services. Their role involves overseeing the quality control processes, identifying areas for improvement, and ensuring that service delivery is consistent and meets regulatory standards. Specific tasks include documentation and reporting; quality assurance; performance improvement; compliance and standards; training; root cause analysis and collaboration among various departments.
RESPONSIBILITIES OF THE JOB AND ESSENTIAL ELEMENTS:DIRECT DUTIES - Coordinate internal investigations; track trends and monitor progress of investigations
- Travel as needed to our Delaware locations
- Prepare and manage Incident Management Reports and Review of Incident Investigations
- Conduct Certified Investigations from commencement to finalization
- Adhere to all investigation, reporting, and filing timelines as specified by Incident
- Management Bulletin and the Certified Investigator course requirements
- Track and assure that all Occurrence Reports are reported in a timely manner
- Issue final Investigation report per regulations
- Assure the witness statements are taken in a timely manner - In person interview
- Assure the alleged victim(s) statement is taken in a timely manner
- Collect all physical and demonstrative evidence
- Assure the Chain of Custody is followed with collection of materials and information
- Assure the Summary of Evidence is accurate and concise
- Assure the Analysis was supported by the summary of evidence
- Respond to questions/provide additional information as requested by administrative entities
- Attend training necessary to conduct investigations
- Align agency policies with State policies
- Other quality improvement or training tasks as needed
MINIMUM EDUCATION AND EXPERIENCE QUALIFICATION REQUIREMENTS: - Labor Relations Alternatives Consultants Training Conducting Serious Incident Investigations Course
- Two (2) years of experience in investigative work or related field
- Bachelor's degree in human services, Criminal Justice or related field
- Knowledge of local, state, and federal human services rules and regulations
- Excellent communication, writing, documentation, and personnel record keeping skills
- Excellent organizational skills and detail oriented
NETWORK FUNCTIONS: - Maintain and abides by OSHA requirements and guidelines
- Observes all Fire and Safety Policies and Procedures
- Observes all Personnel Policies and Procedures
- Observes all Agency's Policies and Procedures
- Observes all HIPAA Policies and Procedures and maintains confidentiality
- Completes and returns all paperwork distributed by the Human Resource Department
- Attends all required and scheduled trainings and meetings
- Always maintain a professional demeanor and exercise good professional judgment in all areas of employment duties
- Documents time worked accurately
EQUIPMENT/MACHINES:EQUIPMENT OPERATIONS: - Telephone, cellular telephone, copier, fax, computer, calculator
COMPUTER SOFTWARE APPLICATIONS: - Computer literacy in MS Office products; Word, Excel, Outlook, Power Point.
PHYSICAL DEMANDS: - Individual must be able to meet physical requirements of the job:

- Must be able to demonstrate competency as a Mandt and CPR facilitator which includes physical skills demonstration
