About myPlace Health

myPlace Health was founded in 2021 by mission-aligned healthcare leaders and organizations that are committed to drastically improving health outcomes, quality and experience for vulnerable older adults and frail seniors. We specialize in providing value-based, comprehensive care and coverage for older adults with significant needs so they can thrive in the homes they love and in the communities they cherish.

Our mission is simple: to enable older adults to live the independent lives they deserve. We pursue this mission through our myPlace PACE (Program of All-Inclusive Care for the Elderly) model, which provides seamless primary care, integrated health plan coverage, personalized social engagement, and customized services delivered in the participant's preferred place.

myPlace Health is building a mission-driven team that shares our passion for redefining the way older adults experience care as they "age in place" in the community. This is a unique opportunity to take on one of our country's most challenging healthcare problems and join a fast-growing, dynamic team as we prepare to scale our mission to serve more markets.

About This Role

If you love bringing smiles to people's faces, finding creative ways to connect, and turning everyday moments into something special-this is the role for you.

As an Activities Assistant, you'll help create a vibrant, joyful environment for older adults enrolled in our PACE program. From morning stretches and music circles to art projects and off-site adventures, you'll support the daily activities that keep our participants engaged, inspired, and feeling part of a caring community.

You'll work hand-in-hand with our Activities Coordinator and a dedicated interdisciplinary team to ensure every activity is safe, meaningful, and tailored to the unique needs of those we serve. Whether you're helping someone get to the game room, prepping materials for a craft session, or simply sharing a warm conversation, your presence makes a difference.

This is a great fit for someone who's reliable, upbeat, and flexible-with a big heart and a sense of fun. If you believe that laughter, creativity, and kindness belong in every day, we'd love to welcome you to our team.

What Does Success Look Like As Our Activities Coordinator?
    • Bringing Creativity to Life - Support the delivery of daily recreational, therapeutic, and social activities that align with each participant's care plan and our overall program goals. Whether it's art, music, movement, or games, you'll help make each day engaging and uplifting.
    • Championing Participant-Centered Care - Encourage and inspire participants to take part in activities that reflect their interests, abilities, and cultural backgrounds-helping them feel included, respected, and empowered.
    • Collaborating as a Team Player - Work closely with the Activities Coordinator and interdisciplinary team members-like social workers, therapists, and clinical staff-to design and deliver experiences that truly support each participant's physical, cognitive, and emotional needs.
    • Offering Compassionate Support - Provide hands-on assistance during group and individual activities and outings, ensuring safety, comfort, and connection for all participants.
    • Staying Organized and Prepared - Set up activity spaces and materials ahead of time so that programs run smoothly and efficiently. You'll help keep supplies stocked, clean, and ready for use.
    • Being a Friendly Companion on Outings - Accompany participants on off-site trips, helping with coordination, safety, and encouragement so that everyone can enjoy meaningful experiences beyond the center.
    • Observing with Care and Intention - Keep an eye on participant engagement, mood, and any changes in condition-then communicate those insights with the team to support timely, compassionate care.
    • Maintaining Accurate Records - Log participant attendance and relevant observations in a clear and timely manner, supporting our program documentation and quality improvement efforts.
    • Creating Memorable Moments - Help bring special events to life, including seasonal celebrations, intergenerational programs, and community engagement activities that bring joy and a sense of belonging.
    • Prioritizing Safety and Cleanliness - Ensure all activities and environments are conducted with safety and infection control practices in mind, contributing to a clean, welcoming, and supportive space.
    • Contributing Fresh Ideas - Share your creativity and suggestions to help us continually grow and improve our program, always keeping participant needs and preferences at the heart of what we do.
    • Stepping in Where Needed - Be flexible, dependable, and ready to lend a hand wherever support is needed-because creating joyful moments is always a team effort.
What Does An Ideal Candidate Look Like?
    • A solid foundation to build on - A high school diploma or equivalent is required. Bonus points if you've taken coursework or earned a certification in recreational therapy, gerontology, or a related field.
    • Hands-on experience with heart - At least one (1) year of experience supporting older adults or individuals with physical or cognitive challenges in settings like healthcare, senior centers, or community-based programs.
    • Comfort leading the way - Experience facilitating individual or group activities is preferred-especially in environments such as PACE programs, adult day health centers, or long-term care.
    • A talent for joyful engagement - You know how to encourage participation and connection in a respectful, uplifting, and person-centered way.
    • A team-first attitude - Strong interpersonal and communication skills are essential, along with the ability to collaborate effectively with a diverse interdisciplinary team.
    • Compassion and dependability - You bring a calm, patient, and flexible spirit, and show up with empathy and reliability every day.
    • Readiness to adapt - You're comfortable working in a dynamic, fast-paced environment, adjusting as needed to meet the needs of participants and the team.
    • Supportive physical presence - You're able to assist participants with mobility safely and confidently, including supporting the use of wheelchairs, walkers, and other mobility aids.
Other Pertinent Information:
