Responsibilities: - Requires one evening per week and every other Sunday.
- Helps resident maintain their highest level of functioning by presenting appropriate activity programs according to resident's physical capacity, cognitive ability and interest.
- Motivate resident to take an interest in their life and community by investigating alternative methods of activities, resident hobbies, and community interest.
- Delivers mail, flowers and other items to residents in timely manner and reads mail to residents upon request.
- Prepares for activity program by assuring all supplies are available and room is ready.
- Assist in transporting residents to activity program.
- Assist resident in craft projects and activity.
- Implements activity procedures Assist directed by the Activity Director by creating decorations, setting up necessary equipment, escorting residents to and from activity area.
- Assist in scheduling movies, planning parties and providing games/activities for residents.
- Supervise activities as necessary.
- Safety and care of resident during transport.
- Safety and care of resident during activities.
- Proper chart documentation or resident participation and evaluation.

- Performs other duties as required.
Qualifications: - Graduate of an accredited high school or GED preferred.
- Previous long-term care experience is desirable.
- Responsible for assisting with the planning, implementation, and evaluation of recreational, social, intellectual, emotional, and spiritual programs for a long-term care nursing center in accordance with patient assessment plans and regulatory guidelines.
- Assists with documentation of patient assessments and activity attendance records.
- Embrace the standards, values, and beliefs of the organization.
- Possess the ability to communicate effectively orally and in writing.
- Be capable of maintaining effective working relationships with patients, family members, department heads, and fellow associates.
Experience Preferred in Skilled Nursing Setting
