JOB TITLE: Prevention Clinical Case Manager

FSLA Status: Non-Exempt

JOB OBJECTIVE: To provide in-person need assessments, crisis intervention, prevention and aftercare case management for youth and young adults in need of Homelessness Prevention services. All services and supports provided for the individual and their family will be provided through a clinical needs-based lens.


ADMINISTRATION

Essential Functions - Overall

  • Complete assessments and intake process for Prevention clients both on site and within the community.
  • Provide individual and/or family Homeless Prevention services for young adults and families who are at-risk of experiencing a housing crisis.
  • Create and assist with support plans for housing stabilization and long-term goal setting with Prevention clients and/or families.
  • Assist youth from a strengths-based case management perspective as they master life skills, included but not limited to: job readiness, communication skills, transportation, budgeting.
  • Utilize creative approaches to provide clinically influenced support plans for youth and young adults on case load.
  • Operate from a family systems approach to support young people in family strengthening efforts, as appropriate.
  • Provide liaison, advocacy and referral services with appropriate mental health centers, courts, physicians, school systems, Department of Children's Services and others as needed.
  • Connect youth, young adults and families to appropriate community resources in an effort to build out their continuum of support as a part of client's aftercare plan.
  • Learn and operate from a trauma informed/positive youth development approach to foster a safe healing environment to enable youth to overcome their life challenges.
  • Help to build and implement strong internal collaborations that maximize Oasis Center's resources for young people.
  • Other tasks as assigned.

Essential Functions

  • Input participant enrollment and program data into multiple databases- HMIS and Salesforce. (weekly).
  • Ensure program data is entered accurately and timely into HMIS and other grant-mandated databases, as assigned.
  • Query HMIS and other databases to monitor program data quality, working with program leadership to resolve problem areas.
  • Work with agency HMIS Point of Contact (POC) to implement all local standards, policies, and procedures.
  • Assist with database training as needed.
  • Consult with supervisor on data collection problems.
  • Participate in staff and program development by attending Oasis Center and community-based trainings.
  • Attend weekly staff meeting and regular supervision, collaborating with supervisor and appropriate staff on specific case-related problems.

Requirements:

MININMAL REQUIREMENTS/PERFORMANCE STANDARDS

  • Proficient in the use of Microsoft Office, especially Microsoft Excel.
  • Experience with cloud base services such as Microsoft 365 and Drobox.
  • Proficient in or ability to gain proficiency in HMIS and Salesforce
  • Experience with database development and/or database management.
  • Strong data analysis competencies.
  • Ability to use logic and to think independently and systematically about data processes.
  • Ability to give and receive feedback within a team.
  • Ability to multitask and bring order to a variety of processes.
  • Ability to effectively set and management multiple priorities and use time constructively.
  • Ability and desire to learn on the job.
  • Ability to form strong, caring and affirming relationships with all youth -including LGBTQ+ youth.
  • Clear understanding of racial inequities and other forms of discrimination and willingness to operate from an anti-oppression model.
  • Knowledge of an ability to apply Positive Youth Development, Trauma-Informed Care, and evidence-informed practices based in an understanding of adolescent development.

Educational Requirements/Performance Standards: Bachelor Degree or three years relevant experience


PHYSICAL DEMANDS

  • Ability to communicate effectively in oral and written form.
  • Ability to utilize office equipment and computer software.
  • Ability to maneuver stairs and uneven terrain.
