We are currently seeking a skilled and dedicated Wound Care Nurse to join our team.

Job Description: As a Wound Care Nurse at Chestnut Hill Lodge, you will play a vital role in the care and treatment of our residents with acute or chronic wounds. You will work closely with the interdisciplinary healthcare team to assess, manage, and treat various types of wounds, including pressure ulcers, surgical wounds, diabetic ulcers, and other skin conditions. Your expertise in wound assessment, treatment modalities, and preventative measures will be essential in promoting optimal wound healing and preventing complications.

Responsibilities:

  1. Conduct comprehensive wound assessments on admission and regularly thereafter, documenting findings accurately in residents' medical records.
  2. Develop individualized wound care plans in collaboration with the interdisciplinary team, incorporating evidence-based practices and current guidelines.
  3. Implement appropriate wound care interventions, including cleansing, debridement, dressing changes, and wound irrigation, while adhering to infection control protocols.
  4. Monitor and evaluate the progress of wound healing, adjusting treatment plans as necessary to promote optimal outcomes.
  5. Educate residents, families, and staff members on wound care techniques, prevention strategies, and the importance of proper nutrition and hydration in wound healing.
  6. Collaborate with physicians, wound care specialists, and other healthcare professionals to ensure coordinated and comprehensive care for residents with complex wound care needs.
  7. Participate in quality improvement initiatives related to wound care, including data collection, analysis, and implementation of best practices.
  8. Maintain up-to-date knowledge of advances in wound care technology, products, and treatment modalities through ongoing education and professional development.
Qualifications:
  • Valid Registered Nurse (RN) license in the state of PA.
  • Certification in Wound Care (CWCA, CWCN, or equivalent) preferred.
  • Minimum of 2 years of nursing experience, with a focus on wound care and management.
  • Proficiency in wound assessment, documentation, and treatment modalities.
  • Strong critical thinking and problem-solving skills.
  • Excellent communication and interpersonal abilities, with the ability to collaborate effectively with interdisciplinary team members.
