Garden Spring Nursing & Rehab Center is looking for a Wound Care Registered Nurse. Utilizing the Nursing Process, the RN plans, implements, directs and evaluates patient care to achieve optimal outcomes. The Wound Care Nurse works collaboratively with all other disciplines on assigned unit. Additionally, the RN prioritizes nursing actions and will implement physician orders accurately within appropriate time frames. POSITION PURPOSE: Responsible for managing the skin program for the facility.

ESSENTIAL FUNCTIONS OF POSITION:

  • Assess all residents weekly to evaluate the condition of their skin and provide appropriate and required documentation.
  • Measure all skin areas weekly and document.
  • Initiate and update all care plan information regarding skin.
  • Follow up with physician to report any resident skin problems that arise, and transcribe and follow up with any new orders.
  • Perform skin and related treatments on all residents as assigned, according to Physician's orders and in accordance with professional standards.
  • Communicate with the physician, residents, family members, charge nurse, other interdisciplinary team members, and facility management regarding resident status as appropriate.
  • Represent the Skin Management meeting on a weekly basis and provide needed information to direct and participate appropriately.
  • Monitor and adjust equipment use per facility protocol and resident need. Keep an ongoing record of all facility equipment and use. Be available for review of new and upcoming devices that can promote healing and reduce risk of infection.
  • Follow skin protocol of facility
  • Be able to provide weekly skin log, utilize computer programs available, and take responsibility for the prevention, healing, and educating of residents, families, and staff in areas of need according facility protocol.
  • Participate in Quality Improvement activities as assigned.
  • Perform additional duties as assigned

EDUCATION / REQUIREMENTS:
RN license preferred. plus a minimum of 3 years related experience;
