Massapequa Center for Rehabilitation and Nursing is a 320-bed skilled nursing and rehabilitation facility located in Amityville NY, our mission is to provide the highest level of care for each of our residents.

As a Wound Care Nurse, you will play a critical role in providing specialized care to patients with acute or chronic wounds. Your primary responsibility will be to assess, plan, implement, and evaluate wound care interventions for our residents, ensuring optimal healing and prevention of complications. The ideal candidate should have a strong clinical background in wound management and be knowledgeable about current wound care practices.

Responsibilities:

-Wound Assessment and Planning: Conduct comprehensive assessments of patients' wounds, considering factors such as wound type, size, location, tissue viability, and underlying health conditions.

-Collaborate with the interdisciplinary team to develop individualized wound care plans, setting appropriate goals and interventions.

-Continuously monitor and document the progress of wounds, modifying treatment plans as necessary.

Wound Care Interventions:

-Perform specialized wound care procedures, such as wound cleansing, debridement, dressing selection, and application.

-Administer topical medications and wound adjunctive therapies as prescribed by physicians.

-Educate patients, families, and caregivers on proper wound care techniques and prevention strategies.

-Ensure compliance with infection control practices and maintain a clean and sterile wound care environment.

Collaboration and Communication:

-Collaborate with physicians, nurses, physical therapists, and other healthcare professionals to coordinate wound care interventions and interdisciplinary treatment plans.

-Act as a resource person for staff members, providing guidance and education on wound care best practices.

-Maintain accurate and detailed documentation of wound assessments, interventions, and outcomes.

-Communicate effectively with patients and their families, addressing their concerns and providing emotional support.

Quality Improvement and Education:

-Stay up-to-date with the latest wound care research, innovations, and evidence-based practices.

-Participate in quality improvement initiatives, ensuring compliance with regulatory guidelines and best practices in wound care.

-Conduct educational sessions for staff members, focusing on wound care prevention, identification, and treatment.

Qualifications:

-Valid registered nursing (RN) license in the state of practice.

-Minimum of 1 year of clinical experience as a wound care nurse, preferably in a skilled rehab nursing home or similar setting

-In-depth knowledge of wound etiology, assessment techniques, staging, and evidence-based wound care interventions.

-Proficient in wound documentation and understanding of electronic medical record systems.

-Strong critical thinking, problem-solving, and decision-making skills.

-Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.

-Ability to work collaboratively within an interdisciplinary team.

-Strong attention to detail and ability to multitask in a fast-paced environment.
