Wound Care Nurse
Looking for a licensed Register Nurse (RN)

The Wound Care Nurse is responsible for providing direct nursing care to residents under the medical direction and supervision of the residents' attending physicians, the Director of Nursing Services, and the Medical Director of the facility. They also monitor the performance of non-licensed personnel and assist in modifying the treatment regimen to meet the physical and psychological needs of the resident, in accordance with established medical practices and the requirements of the policies and procedures of this facility in accordance with current federal, state, and local standards governing the facility.

Job Responsibilities:
  • Stages wounds and provides charting on wound care initiatives
  • Provide excellent nursing care to patients suffering from wounds.
  • Assess and evaluate patients with wounds and injuries.
  • Obtain cultures to assess wounds and injuries.
  • Evaluate wounds and injuries for infections or other illness factors that causes wounds.
  • Initiate nursing care procedures in managing acute and traumatic wounds.
  • Coordinate with rehab, traumatic and nutritional nurses in nursing patients with wounds.
  • Ensure optimum patient care delivery in wound care nursing procedures.
  • Provides direct resident care as needed.
  • Review the resident's chart for specific treatments, medication orders, diet, etc., as necessary.
  • Educates the resident through the use of nursing knowledge and skills according to their needs to promote their mental and physical well-being.
  • Assist with physician rounds and examinations.
  • Consults with resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.
  • Responsible for interpretation and execution of physician's orders and calling physician as indicated.
  • Assists with checking monthly orders with MAR's and TAR's.
  • Completes MDS and Care Plans as assigned.
  • Reviews care plans to ensure that appropriate care is being rendered.
  • Is responsible for accurate observation, evaluation, and reporting of resident's symptoms, sudden change in weight, change in condition, reactions, and progress to physician and shift supervisor.
  • Perform other duties as assigned.
Education and Qualifications:
  • Graduate from an accredited school of nursing.
  • Must possess a current, unencumbered, active license to practice as a RN in Pennsylvania
  • Previous long-term care experience is desirable.
  • Computer knowledge preferred.
