23-24 WIC Resource Navigator Join Sixteenth Street Community Health Centers (SSCHC), a federally qualified community health center in the heart of Milwaukee's southside. Member Duties : The WIC Resource Navigator AmeriCorps Member will be serving as the Women, Infant and Children (WIC) Resource Navigator to increase services offered to our WIC families by educating WIC participants in navigating and successfully connecting to needed community resources. Ensure appropriate referrals and made Familiarize self and others with community resources. Educate patients about self-advocacy Connect with participants via phone or in person for referral follow up. Collect and assess patient data relevant to needs, barriers and/or behavior change. Collect and report data of individuals served on a change of behavior or intent to change behavior for a healthier lifestyle.
