Who you are:
  • Minimum LPN/RN degree in Nursing required.
  • Minimum on (1) year experience in Nursing experience in assisted living, memory care preferred.
  • The ability to speak, read and comprehend the English language
  • Knowledge in all areas of local Health Department, OSHA, and other regulatory agencies relevant to healthcare services
  • Good communication skills (oral and written)
  • Good inter-department communication and teamwork skills
  • Capable of administering employee incentive, retention, and training programs
  • Familiarity with Microsoft Office Suite products

General Summary:

The Wellness Nurse provides direct nursing care to residents and delegates and supervises the care and services provided by Quality of Life Specialists. Assist the Director of Health Services in ensuring community adherence to all applicable federal, state, and local standards and regulations promoting regulatory compliance.

Essential Job Duties:
  1. Document resident status as per policy.
  2. Provide daily supervision to assigned staff and directly provide nursing care to residents as assigned.
  3. Share weekend, afterhours, and holiday on call responsibilities with Director of Health Services.
  4. Participate in the admission of new residents.
  5. Assist residents with Activities of Daily Living (ADL), family/social activities and actively promote independent life-style of residents.
  6. Responsible for identification of resident medical and behavioral changes, cognitive assessments, and the development and monitoring of care plans and re-assessment of each resident.
  7. Provide daily report to the Director of Health Services concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies.
  8. Participate in daily meetings in the absence of the DOHS.
  9. Provide nursing care according to physicians' orders and in conformance with recognized nursing practice, established standards, and administrative policies. Document resident status as per community policy.
  10. Assist the DOHS with the auditing of resident records to ensure adherence to community policy and procedures and regulatory compliance.
  11. Attend Quality Assurance, Round Table or Care Conference meetings as designated by the DOHS or Executive Director.
  12. Ensure preventive, therapeutic, and rehabilitative services to residents and assist in the coordination of nursing care with care rendered by other disciplines.
  13. Make rounds with physicians; assist in examinations and/or treatment of residents and carry out orders.
  14. Assess resident and notify attending physician and responsible party in the event of a change in the resident's condition,
  15. Accurately administer prescribed medications, supervise the continuity of nursing care, respond to changes in medical condition, medications or other circumstances requiring special care needs and perform first aid as required.
