Purpose of Your Job Position

As Wellness Coordinator at Marymount Place Assisted Living, you will utilize your leadership qualities to inspire, lead and manage the overall operation of the clinical team to provide the highest quality of care and services for our residents. You will proactively build relationships with residents, families, physicians and other healthcare providers for the coordination of exceptional personalized care. You will consistently collaborate with community leadership, mentor and engage your associates and build resident and family satisfaction.

Delegation of Authority

As the Wellness Coordinator your direct supervisors are the Assisted Living Administrator and the Director of Nursing. You are responsible for supervision of the Nurses and Care Attendants.

Major Duties and Responsibilities

1. Make visits to hospitals, rehab facilities, other assisted living communities, private homes to assess future residents.

2. Represent and market Marymount Place Assisted Living to all professionals and potential referral sources in the field.

3. Tour and encourage potential residents to join Marymount Place Assisted Living when needed.

4. Assist the Administrator on sales calls or tours when needed.

5. Assist the Administrator and DON in planning, developing, organizing, implementing, evaluating, and directing the Assisted Living functions in accordance with current rules, regulations, and guidelines that govern the facility.

6. Assist the Administrator and DON in implementing the appropriate plans of action to correct identified problem areas post survey and through quality assurance and improvement processes.

7. Provide feedback to the Administrator and DON regarding resident care and operational concerns.

8. Assist with the development and maintenance of written policies and procedures that govern the clinical operations of the facility.

9. Review and monitor monthly financial statements and provide variance summary to the Administrator.

10. Assist in the preparation of the annual budget process as needed.

11. Assist in the investigation and resolution of any resident grievances that occur.

12. Participate in Resident Council as needed.

13. Work closely with other campus departments to ensure that residents are safe and satisfied, and that all assisted living rules and regulations are met.

14. Complete performance reviews for all nursing staff at Marymount Place.

15. Assist in survey (inspection) made by the authorized government agencies.

16. Serve in capacity of Care Manager as needed.

17. Schedule, coordinate, attend, and participate in Care Plan meetings as needed.

18. Perform administrative duties such as completing assessments, medical forms, reports, evaluations, studies, charting, etc. as necessary.

19. Teach and lead staff to perform their job duties within Federal, State, industry-wide guidelines and practices as well as those established safety regulations in the use of equipment/supplies.

20. Assist the DON in the recruitment and selection of nursing personnel.

21. Scheduling staff according to budget requirements for each shift to ensure that quality care is maintained. Monitor absenteeism to ensure that an adequate number of staff are always on duty.

22. Develop methods of coordination of nursing services with other resident services to ensure the continuity of each resident's total regimen of care.

23. Ensure that disciplinary action is administered fairly and without regard to race, color, creed, national origin, age, sex, religion, veteran status, handicap, or marital status in accordance with company policies.

24. Assist with the implementation and maintenance of an effective orientation program that orients the new employee to the facility, its policies and procedures, and to his/her job description.

25. Assist in conducting mandatory in-services for staff ensuring that all staff attend and receive the appropriate in-services via State and Federal regulations.

26. Make daily rounds of the Nursing Department to ensure that all nursing personnel are performing their work assignments in accordance with acceptable nursing standards and that residents are being care for according to establish service levels and needs.

27. Assist with the implementation and maintenance of the resident admission and discharge policy for the facility.

28. Ensure that nursing staff are prepared and rooms are ready for the new resident.

29. Review the nurse's notes to ensure that they are objective and descriptive of the nursing care being provided and that they reflect the resident's response to the care.

30. Encourage attending physicians to record and sign progress notes, physician's orders, etc. on a timely basis and in accordance with current regulations.

31. Assist in the completion census information.

32. Assist the DON in the development, implementation, and maintenance of a program for monitoring communicable and/or infectious diseases among the residents and facility staff.

33. Assist the DON develop, maintain, and periodically update infection control precautions and aseptic techniques that successfully correct problem areas.

34. Assist in monitoring infection control safety issues including equipment and supply storage.

35. Consult with housekeeping on infection control procedures for the handling of clean and soiled laundry/linen.

36. Monitor medication passes/treatments to assure that appropriate hand washing techniques are followed in the handling and administration of drugs, medications, and treatments.

37. Coordinate and oversee the annual resident flu vaccine program for Marymount Place.

38. Evaluate residents' care needs prior to admission and as condition changes with input from the resident, family and staff.

39. Participate in and promote Quality Assurancet/Performance Improvement Program as required.

40. Evaluate the effectiveness of the Plan of Care and assist in documenting changes and/or revision to the plan.

41. Assist in coordinating and implementing the individual therapy needs for each resident outlined by the physician's orders and resident care plan.

42. Ensure that all nursing staff are in compliance with residents' rights.

43. Assist in reporting complaints and grievances made by the resident and actions taken to resolve the complaints or grievances.

44. Ensure that all resident care rooms, treatment areas, etc. are maintained in a clean, safe, and sanitary manner.

45. Assure that all facility personnel, residents, visitors, etc. follow established safety regulations to include fire protection/prevention, smoking regulations, infection control, etc.

46. Maintain confidentiality of all resident care information.

47. Assure that all residents receive care in a manner and in an environment that maintains or enhances their quality of life and care.

48. Assure that each resident received the necessary nursing, medical, and psycho-social services to attain and maintain the highest possible mental and physical functional status.

49. Create and maintain an atmosphere of warmth, personal interest, and positive emphasis as well as a calm environment throughout the facility.

50. Maintain a good rapport and act as liaison with families and residents.

51. Attend and participate in continuing education programs designed to keep abreast of changes in profession as well as to maintain licensure on a current status.

52. Attend required in-services regarding various topics and Corporate Compliance.

WORKING CONDITIONS:

Works in well-lighted / ventilated areas.

Subject to frequent interruptions

Communicates with the medical and nursing staff, and with residents and families.

Willingness to work beyond normal working hours when necessary.

This position involves tasks which may result in exposure to blood, body fluids, tissues, or to supplies or equipment contaminated with blood, body fluids, or tissues. The following list includes but is not limited to those tasks during which you are likely to come into contact with blood, body fluids, or tissues:

Performing any task in the room of residents who are spraying blood or body fluids, e.g., coughing and expectorating secretions.

Performing blood glucose testing and treatments on any non-intact skin areas or mucous membranes.

Providing or assisting with personal hygiene to non-intact skin areas or mucous membranes or to areas soiled with blood or body fluids.

Handling of linens, dressings, equipment, or supplies which are soiled with blood, body fluids, or tissues.

Giving injections and handling of used sharps, such as needles, IV catheters, instruments, lancets, etc.

Inserting, removing, or handling and cleaning of any tubes, e.g. tracheostomies, feeding tubes, etc.

Obtaining and transporting specimens for laboratory analysis to the lab refrigerator.

Assisting with procedures which may cause drainage or bleeding such as debridement, dental procedures, etc.
