Position Description :

To ensure that members are receiving services as scheduled and needs are being met along with providing members with necessary resources and education that will assist the member in remaining in the least restrictive environment.



Essential Job Functions:

  • Establish and maintain a positive working relationship with members, encouraging them to utilize planned services to their optimal benefit.
  • Complete home visits as required and as needed to review ongoing services as well as ongoing care plan for identified/continued needs.
  • Provide the member with information on community resources and agency services which will address their needs and make referrals as needed on behalf of the member.
  • Utilize skills, such as advocacy, monitoring, and discharge planning to link members and their families to resources and services.
  • Monitor service delivery for effectiveness and efficiency on an ongoing basis and ensure member/caregiver satisfaction with all services.
  • Educate the member/caregiver within the scope of knowledge by providing health education or information about community resources.
  • Complete Event Based Visit process including phone call and/ or home visit with member/caregiver due to a hospitalization or NF admission or other significant event to provide education and reduce risk of readmission.
  • Empower and motivate members to gain access to health and community resources through education, outreach, home visits, mentoring, and referrals.
