Job Summary:

The Unit Manager for Elevate Care is responsible for overseeing the daily operations of a designated unit within our facility. They are tasked with managing a team of nursing staff, ensuring high-quality patient care, maintaining regulatory compliance, and fostering a supportive and efficient work environment. The Unit Manager collaborates with other healthcare professionals to develop and implement care plans, address patient needs, and maintain a safe and comfortable environment for residents.


Responsibilities:

  • Supervise and lead a team of nursing staff, including registered nurses, licensed practical nurses, and certified nursing assistants.
  • Coordinate and oversee patient care activities, ensuring adherence to established protocols, standards, and regulations.
  • Develop and implement nursing care plans in collaboration with physicians, therapists, and other healthcare professionals to meet the individual needs of residents.
  • Monitor and assess the quality of care provided to residents, identifying areas for improvement and implementing corrective actions as necessary.
  • Conduct regular rounds to assess resident conditions, address concerns, and provide support to staff members.
  • Ensure compliance with regulatory requirements, including state and federal regulations, as well as facility policies and procedures.
  • Participate in staff recruitment, training, and performance evaluation processes to maintain a competent and engaged workforce.
  • Foster a positive and supportive work environment, promoting teamwork, communication, and professional development among staff members.
  • Collaborate with other departments within the nursing home facility to coordinate services and resources for resident care.
  • Serve as a liaison between residents, their families, and healthcare providers, addressing concerns and providing information as needed.
  • Maintain accurate and up-to-date documentation of patient care activities, including assessments, care plans, and progress notes.
  • Respond to emergencies and crises, providing leadership and direction to staff members to ensure resident safety and well-being.
  • Participate in quality improvement initiatives and interdisciplinary care conferences to enhance the overall quality of care provided to residents.
  • Stay informed about developments in nursing practice, regulations, and healthcare trends, incorporating best practices into unit operations.

Qualifications:

  • Current, unrestricted Registered Nurse (RN) license in the state of Illinois.
  • Minimum of 3-5 years of nursing experience, with at least 2 years of supervisory or leadership experience in a healthcare setting.
  • Strong clinical knowledge and expertise in geriatric nursing, long-term care, and rehabilitation services.
  • Excellent leadership, communication, and interpersonal skills.
  • Demonstrated ability to prioritize and manage multiple tasks in a fast-paced environment.
  • Ability to work collaboratively with a diverse team of healthcare professionals.
