SCHEDULE: Monday-Friday 8am-5pm; 4 days remote, 1 day in Davis

This is a temporary position with a duration of up to 6 months

JOB SUMMARY: The Triage RN is responsible for telephone triage and advice services for CommuniCare+OLE patients and community partners (such as hospitals, urgent care centers, pharmacies, laboratories, diagnostic imaging centers, and other health care offices). The RN is responsible for answering the nurse triage line, which may include, but is not limited to, performing an assessment of patient symptoms, and referring them to the appropriate level of care (home care, appointment with primary care provider (PCP), and/or escalating treatment to higher level of care like the emergency department). The RN will collaborate with case management, nursing teams, providers, and other departments to ensure quality patient care. The RN will conduct telephone triage following established procedures and protocols, and document calls as required.

EDUCATION, EXPERIENCE, TRAINING

1. Bachelor of Nursing degree from an accredited nursing school preferred.

2. Minimum two (2) years of experience with direct patient care.

3. Current State of California Registered Nurse License required.

4. Must certify and remain current in CPR or BCLS certification.

5. Experience in a community healthcare center preferred.

6. Outpatient primary care setting experience preferred.

7. Training in motivational interviewing preferred.

8. Strong verbal and written communication skills; Bilingual (English/Spanish) language preferred.

9. Strong interpersonal and customer service skills required.

10. Strong computer and data entry skills required; electronic health record system usage required.

11. Strong analytical and problem-solving skills required.

DUTIES AND RESPONSIBILITIES

1.

Monitors and responds to triage calls same day as scheduled by the call center

2.

Talks directly to patients on the telephone, by protocol performs an appropriate assessment, provides patient education and makes recommendations on next steps

3.

Provides appropriate home health advice to patients who do not need to go directly to the emergency department.

4.

Arranges appointments for patients who do not need to go to emergency department but need to see a PCP. Consults with PCP for clinical questions or other care team members as needed. Is responsible for placing patients in same day appointments or standby appointments for the day and the week.

5.

Recommend patients go to the emergency department for care, as appropriate

6.

Ensures accurate documentation of all consultations and treatments and records them in the patients' record via the electronic health record (EHR) system in a timely manner.

7.

Prioritizes work tasks and services and completes tasks in a timely manner

8.

Assist Supervising Nurse with developing nursing policies and procedures and protocols, as needed

9.

Provides patient education regarding disease processes, therapies and healthful behaviors

10.

Reduce conflict and increase patient satisfaction through timely response to calls

11.

Perform other duties as assigned
