State license not required MUST HAVE DOB & FULL SS at time of submission Please note that we have been instructed not to sign on any travelers that live within 75 miles of Houston Medical Center. Our address is: Houston Medical Center 1601 Watson Boulevard Warner Robins, Georgia GA 31093. We can now accept a candidate within 50-74 mile radius from hotel. But, the rate will be reduced $20
