Responsibilities The Travel Home Health Nurse (RN) provides high-quality in-home health care as a full-time employee to our patients in agencies across 38 states, from California to New York. Responsibilities include but are not limited to : Works to reduce unnecessary patient hospitalizations, improve patient safety, and implements processes and best practices to ensure positive patient outcomes. Makes the initial and/or comprehensive nursing evaluation visit, determines primary focus of care, develops the plan of care within State specific guidelines, and submits accurate, complete, and timely documentation, per policy. Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care. Education and Experience License Requirements Must have 1 year of recent Home Health Registered Nurse experience in the last 12 months Must have 1 year of recent Homecare Homebase (Point Care) experience in the last 12 months.
