Address: 6 Alethia Drive, Storrs, CT 06269 Position Description: 32 Hours per week- 13- week contract (potentially 26) only submit candidates who are open to extending/26 weeks if needed- Outpatient Physical Therapist, Locals are accepted (75 miles or less) at lower BR, All time off required at time of submission. - IVS to review, Manager will interview and offer. Shift: 8H Days Variable Monday-Friday: 8am-4:30pm/9am-5:30pm/7:30am-4pm, no weekends or on-call. Minor holidays are required. EMR: EPIC. Scrubs: any color. Required Upon Submission: 2 years' experience in Outpatient PT. Must have General Orthopedic, post-up, spine experience. AHA BLS for providers; OCS preferred. Active PT License. Preferred: LSVT Certified, Board Certified Neuro Clinical Specialist Required Upon Start: Active CT license is NOT required at time of submission, but they will need to get before they start. Covid Vaccine- Medical and religious declinations are allowed. Flu Vaccine- Medical and religious declinations are allowed. N95 Fit test required. Guaranteed Hours: Contract Weeks:91
