Case Manager needed for Home Health
5-8s weekends in rotation - about 3-4 on average for a standard 13 week contract
Must have OASIS and previous Home Health experience
The Nurse Care Manager I coordinates the care and service of selected patient populations throughout the acute care episode, in order to facilitate optimal transitions of care. Using a biopsychosocial approach, the role works collaboratively with inter-disciplinary teams, both internal and external to the organization, to enhance the quality of clinical outcomes and patient satisfaction while managing healthcare resources.
Education
Graduate of an accredited nursing school. BSN preferred.
Experience
A minimum of three years registered nursing experience in a clinical setting is preferred. Nursing experience in an acute care setting is beneficial. Comparable education and experience may be considered.
Licensure/Credentials
Current Washington State RN license. BLS certification
WA RN license required, Experience with EPIC preferred. Case management and prior acute care experience needed.
18 to 20 patients per day.
