RN Home Hospice
Shift: Days, M-F 8a-4:30p, weekends per rotation
Term: 12 weeks
Certification: BLS
Pertinent Info: weekend rotation (currently every 5rd weekend, but could change), case load of 13 patients, may need to cover a holiday this term.
JOB SUMMARY
Is responsible for making home visits to assigned patients requiring skilled intermittent professional nursing services in the home. All findings and/or changes regarding patient are communicated to the following physician.
MINIMUM QUALIFICATIONS
Minimum Qualifications include: Valid state driver's license, Minimum of eighteen (18) years old, Have an acceptable Motor Vehicle Report (MVR), Must provide proof of insurance, if applicable - Associate's Degree
in Nursing. Current RN licensure in the state of Ohio
3 years home hospice experience preferred

Local Rule: FOR ALL LOCATIONS - The traveler's permanent address must be greater than 100 miles from the most central facility OhioHealth Grant, 111 S Grant Ave, Columbus, Ohio 43215 to be eligible to apply.

Travelers are first to float
Return Staff Policy : If an RN has worked at an OhioHealth facility as staff they need to be gone 366 days prior to being considered as a traveler. PRN at OhioHealth cannot accept a travel position and work both
For travelers, more than one call off is grounds for termination. NCNS is grounds for immediate termination as well.
