In addition to being medically complex discharges patients may have additional needs complicating the discharge such as psych diagnosis, requiring trach and vent management or be sex offenders.Mississippi is not a Medicaid expansion state so does not cover LTAC or skilled which increases the challenge in placing the complex patients upon discharge. UMMC has contracts with 5 local facilities with 10 beds reserved at each facility to assist with the complex placements, however specific criteria must still be met. Criteria has a limitation on financial assistance for only those uninsured or underinsured. Experience or 'Must have' skillsMust be RN Case manager with 5 years Acute Care Management experience with previous experience in large academic Medical Center hospital UMMC is the only level 1 trauma hospital in the state so will receive a variety of complex patients and those with no other hospital options.CM Manager/SupervisorDepartment does not have a CM Manager at this time. Will directly report to the Social Work Manager. Chief of Care Coordination is also providing department oversight as needed.
