ID: Shift: FT, Monday-Friday, 9am-5:30pm; possibility of extending after 8 weeks Description: POSITION SUMMARY The Social Worker position assists and supports patients and their families to identity areas of need. Works with the Case Manager of inpatient services to assist patients and their families with deESSENTIAL JOB DUTIES Assists with discharge planning primarily in Acute Care. Work with IDT to identify needs. Discuss/arranges options, services, and resources to help with areas of need. Assist with Health Care Power of Attorney documents and other Advanced Directives as able. Complete needed, required, and professional documentation and paperwork for documentation of patients' charts and other jobs tasks and requirements.
