Job Title: Physical Therapist
Specialty: Home Health (Adults & Geriatrics)
Salary: $86k-$125,000 per year (not per visit).

Schedule: Monday - Friday (Full time)
Hours: No Calls, No Weekends (unless needed)
Bonus: Relocation Assistance is available

Must have:

• Physical Therapy License in the State of Illinois.
• American Physical Therapy Association (APTA)
• CPR certification per American Heart Association guidelines is required within 30 days of hire.
