Scope of work As Supervisor, provides day-to-day operations support and supervision to the Care Transition Consultants; leading and coordinating referral through-puts while challenging the team to achieve excellence. Responsible for consultant service delivery. As Consultant, obtains and prompts conversion of referrals to admissions and facilitates a smooth transition of patients into Continuing Care facilities. Maintains communication with referral sources, external case managers and insurance companies, and assists with the coordination of services. Provides input to the Vice President, Operations regarding budget, referral/admission barriers and strategies, staffing and customer service. Establishes, monitors and evaluates the performance goals of the liaisons. Encourages individual and team growth among the staff while providing high quality, compassionate service. Approximately 50% of the supervisor's time will be spent in performing supervisory and compliance functions with the remaining 50% spent as liaison with direct, assigned responsibility. The position may transition with growth to a position requiring 100% of time be spent in either role. Must be capable of traveling to and from referral sites located off campus. Qualifications Associate's Degree or equivalent in nursing, therapy, or social work Required.
