Day - 08 Hour (United States of America) Job Summary Provides appropriate intervention services and discharge planning to patients with substance use disorders within the Emergency Department of the Stanford Health Care Tri-Valley hospital. Conducts initial brief assessments, introduces patients to substance use treatment programs and services, expedite appointments at Medication Assisted Treatment-capable clinics, serves as the primary coach for their clients, and maintains ongoing contact with their patient panel. Assist with access to other services such as financial counseling, primary care, mental health services, social services, and residential treatment facilities. Employees may also perform other duties as assigned. Communicates with hospital staff daily during weekdays to recruit patients and provide support to staff around Medication Assisted Treatment (MAT) and substance use disorder treatment: Monitors the ED and inpatient patient tracking systems to screen for eligible patients; checks-in with ED and inpatient clinicians and nursing staff to screen for eligible patients. Conducts substance use assessments for patients in the emergency departments and inpatient units. Utilizes motivational interviewing to support clients in developing individualized action plans. Connects patients with MAT providers, substance use treatment providers and behavioral health providers. Assesses and addresses any barriers to attending follow up appointments (transportation, contact phone availability for appt reminders, shelter needs, etc.). Engage social services to assist with any of these issues as needed. Arranges transportation to nearby residential treatment facilities and partner programs. Assists with navigating barriers to patients obtaining buprenorphine prescription from pharmacy (insurance status, copay expense, cost differences between formulations, etc.). Routinely assists patients by having the patient and/or pharmacist call from the pharmacy to sort out encountered insurance barriers in real time. Schedules follow up appointments, offers additional resources, networks to help achieve sustained access to care. After discharge from ED/inpatient and on the day prior to follow up appointment, contact patients to remind them of their follow up appointment. Supports patient access to the most optimal level of care available. If patient is from out of county and needs to access MAT in their home county, assists to connect with local resources in their home county. Develops a comprehensive referral process to follow up with patients who are in the ED after-hours and who are referred to Substance Use Navigator Initiates, develops, and maintains constructive relationships with community MAT treatment providers. Tracks patient data related to SUN role. Job Qualifications Education High School Diploma or GED required College degree preferred Experience 3 to 5 years of progressively responsible and directly related work experience License/Certifications BLS - Basic Life Support CADC - Certified Alcohol and Drug Counselor Knowledge, Skills, and Abilities Substance Use Navigator Physical Demand and Working Conditions The Physical Requirements and Working Conditions in which the job is typically performed are available from the Occupational Health Department. Base Pay Scale: Generally starting at $31.00 - $40.00 per hour The salary of the finalist selected for this role will be set based on a variety of factors, including but not limited to, internal equity, experience, education, specialty and training. This pay scale is not a promise of a particular wage.
