Job Description

Assists to case manage intermittent nursing care for patients in the home. Participates in patient and family teaching, develops and re-evaluates the nursing plan for the patient formulating nursing diagnosis, goals and priorities. Provides nursing services, preventative measures and palliative procedures. Reports symptoms, reactions, medications and changes in the patient's physical or emotional condition to the appropriate person. Monitors the patient's care for a variety of needs. Provides appropriate referrals. Develop and maintain a nursing plan of care. Demonstrates professional skills, maintains infection control measures, documents thoroughly and provides follow-up.

Qualifications

Graduate of an accredited school of nursing with current and active RN licensure in Indiana and/or Illinois. One year of acute care experience is preferred. Additionally preference given to those with one-year of home care or hospice experience. Must maintain current CPR certification. Must also maintain current driver's license and liability insurance.
