CentraCare Senior Transitions - Nurse Practitioner or Physician Assistant - Transitional Care Unit CentraCare Senior Transitions offers whole-person primary care in assisted living communities, transitional care units, memory care, and long-term care settings. The goal of our provider and nurse teams is to care for patients in whatever senior community you call home. This role will provide rounding services in four transitional care units across the St. Cloud area. Patients served may be recovering from an illness, injury or surgery and need recovery assistance to help them heal safely and rehabilitation therapies to regain strength and independence. Patients usually spend fewer than 21 days in a TCU. We are seeking a full-time provider to care for patients in St. Cloud, Sauk Rapids, and Sartell. Our team coordinates seamless care by working with building staff, community home care agencies and by offering lab, imaging, and pharmacy support services when and where needed. Role Description: Provide on-site, provider rounding for patients residing in transitional care units Full-time providers care for patients Monday-Friday Role will round at one site per week on between 10-17 patients per day Develop and implement care plans to support patients recovery and return to home Support from scheduling and coordination assistants and clinical support from LPNs and RNs Collaborate with community partners within home care, hospice, and other resources as appropriate Schedule: 0.8 FTE - Eligible for full time benefits 4-day work week Call - 1 week/month with no requirement to go in Requirements: Graduate of an accredited Nurse Practitioner Education Program or Physician Assistant Education Program Board Certified/Board Prepared through AANP, ANCC, or NCCPA Licensed or ability to obtain licensure through the MN Board of Nursing Ability to obtain and /or maintain DEA certification for the State of Minnesota Proven ability to communicate effectively with strong verbal skills Excellent interpersonal and customer service skills Must have a valid driver's license and minimum state-required liability auto insurance; local travel is required CentraCare CentraCare is committed to the patients and families we serve in the communities we call home throughout Central, West Central and Southwestern Minnesota.
