Position: Sr. Director, Pharmacy Performance The Sr. Director will promote and refine Monogram's comprehensive medication therapy management (MTM) program consistent with evidence-based guidelines and national quality standards which fosters appropriate medication use through collaboration with internal and external medical teams serving our patients. The Sr. Director will collaborate with pharmacy team members and work with Monogram clinical and operational departments, and Monogram client-facing teams as well as client sponsors to promote appropriate drug therapy and document improved patient medication outcomes. The Sr. Director will ensure integration of evidence-based medication practices, national quality standards and AI technology into Monogram's care model and assist Monogram regional and local physician-led teams execute on MTM deliverables for our clients and patients. Roles and Responsibilities Establish and manage performance and reporting of MH pharmacy quality programs consistent with NCQA/HEDIS, Medicare Stars and Pharmacy Quality Alliance guidelines. Work collaboratively with Pharmacy Performance Leads and pharmacist/POD team members to support MTM activities consistent with quality metrics. Align programs and reporting with client deliverables. Work collaboratively with MH teams to deploy pharmacy quality programs and metrics. Participate in external interdisciplinary care team meetings and produce MTM related documentation to share with appropriate treating providers and clients. Create and maintain job aides and internal and external materials on medication therapy management and pharmacy quality programs. Monitor and refine evidence-based comprehensive medication management activities and pharmacy quality efforts. Manage alignment of the Pharmacist Guide tool and other clinical resource documents with clinical evidence and MH historical data focused on providing actionable and measurable drug therapy recommendations to providers that improve MH patient health outcomes. Create, deploy and manage medication adherence and care gap programs and training materials for pharmacists (and other Monogram clinical staff) consistent with evidence-based guidelines, best practices, and national quality standards. Create, deploy and maintain job aides and educational materials for pharmacy technicians as needed to support pharmacist efforts as requested by pharmacy leadership. Work with medical economic and data analytics teams to identify and deploy AI technologies to improve data capture and improve efficiency of drug therapy problem identification and resolution. Support leadership in demonstrating value of Monogram pharmacy program. Prepare and present client facing materials on MH MTM activities and outcomes. Assist with development and refinement of medication recommendation outcome reporting. Assemble data regarding categorization and clinical outcomes of pharmacy interventions for internal and external use. Continue alignment of pharmacy interventions with validated quality clinical performance measures and Monogram polychronic condition and CKD/ESKD patient management priorities Support Monogram leadership and clinical departments as necessary with any tasks required to deliver excellent personalized polychronic condition management and kidney care and perform all other duties as assigned. Oversee and uphold the mission and values of Monogram Health in all respects. Position Requirements Office-based and located in Brentwood, TN or Tempe, AZ with some domestic travel. Bachelor's degree in pharmacy or PharmD Current pharmacist license 5+ years previous experience (or equivalent) actively practicing as a pharmacist, preferably within a managed care, ambulatory care, or clinical practice environment. 2 + years' experience with health plan pharmacy management, pharmacy quality program, and pharmacy data analysis. 1+ year experience with clients and vendors related to pharmacy & medication management programs. Desired qualifications include: PGY1 residency in ambulatory care, managed care, or related clinical field. BPS board certification in pharmacotherapy, geriatrics, ambulatory care, or other clinical field. Board certification in MTM MTM certificate training from APhA, Powerpak, or similar program Proficient in excel and other pharmacy data reporting tools.
