Description
Provides education and support services to patients, families and staff. Provides psychosocial assessment, counseling, crisis intervention, referrals and information to patients and families. As a member of the Case Management Department, works collaboratively with the interdisciplinary team to affect positive patient and family outcomes. Serves as resource for less-experienced social workers and other team members.

Required qualifications:
- Master's degree in Social Work from an accredited graduate school of social work.
- Texas Masters Social Worker License - Texas State Board of Social Worker Examiners upon hire or
- New Mexico Masters Social Worker License - New Mexico Board of Social Work Examiners upon hire
- 1 year of Minimum of one year experience delivering social work services in a healthcare setting.
Preferred qualifications:
- Post degree experience in a hospital or other health care setting.
