Job Description: The Role: Social Worker (LSW) - Gardiner Family Medicine The Opportunity: MaineGeneral Health is offering an exciting opportunity for an individual to join our Gardiner Family Medicine team as a full time Social Worker. If you have a passion for service excellence and want to work among excellent colleagues we want to hear from you. This position is 40 hours a week, Monday through Friday, with daytime hours. The Work: Provides case management and coordination for patients and/or families needing a variety of educational, social, and advocacy services. Researches, develops, and delivers educational materials related to patient and family support based on learning styles and needs of populations served. Provides assistance to patients in developing support systems, linking patients and families to community resources as appropriate. Works collaboratively with staff, payers, and external resources to identify needs, cultivate relationships with internal/external resources, and enhance clinical outcomes.
