This position will:
- Review new admissions for any special needs or issues that need to be addressed
- Assist with discharge planning and coordination of transition of care
- Provide and help patients understand financial, emotional, and social resources available to them

Other Requirements:
- Bachelors Degree in Social Work
- Wisconsin Social Work license
- Medical social work experience preferred, but not required
