Description:
Burnett Medical Center, located in Grantsburg, Wisconsin, is seeking a Social Services Manager.
SHIFT & HOURS: 0.8 FTE or 32 hours a week, day shift primarily, with possible occasional evening/weekend availability needed.
JOB SUMMARY
We are searching for a candidate who would manage our Social Services department. They would provide assistance and expertise in areas of patient/resident care, discharge planning and home planning processes with other facility departments, external organizations, agencies and health care facilities. They would ensure compliance, quality care and regulatory standards.
Requirements:
QUALIFICATIONS
-BSW or MSW from an accredited school of social work.
-Wisconsin licensure required.
-Prefer social work experience in a hospital and/or a long term care facility.
-Positive attitude, strong people skills with internal and external customers, strong verbal and written communication skills, ability to adapt to change and to be a team player.
-Knowledgeable of community resources and able to network within the community a plus.
