Role description The mission of JHS is to provide comprehensive, compassionate, and respectful care for incarcerated people while mitigating the effects of structural racism and historical injustices, as a fully integrated and collaborative member of San Francisco's Public Health Department. JHS is responsible for the care of over 1,000 individuals on any given day, and more than 8,000 individuals over the course of the year. Our team of eight Nurse Practitioners and eight physicians provides a unique blend of urgent care including infectious disease management and substance withdrawal, as well as full-scope primary care including chronic disease management, health promotion and disease prevention. Our patients are 91% male, 41% Black and 27% Latinx, 35% unhoused, 79% under 45 and 1% over 65, and a majority have a substance use disorder and/or a serious mental illness. Under the general supervision of the Chief Medical Officer, the Jail Health Services' Complex Care and Population Health Manager will serve as a clinical leader for healthcare staff and an advocate for patients. They will be responsible for coordinating the care of complex patients in the jail and preparing them for release into the community. This physician leader will also oversee population health improvement efforts across Jail Health. Communication and collaboration with Sheriff's Department staff is critical. Duties Include Provide full-scope primary care, particularly for complex patients (60% of the time): Collect medical histories, review outside records, perform physical examinations, interpret and evaluate diagnostic tests, and establish treatment plans; counsel and educate patients regarding both preventive care and chronic diseases; manage substance use disorders; refer patients to specialty care when appropriate; document patient information in the electronic medical record. Design and implement population health improvement efforts (20% of the time): potential examples include interventions to increase the rate of TB screening, colon cancer screening, and smoking cessation post-release. Create workflows and systems to improve the transition from jail to community healthcare (20% of the time), in collaboration with reentry, discharge RN, and other members of the Jail Health team. Participate in and lead clinician staff meetings related to complex patients and population health. May supervise the work of other healthcare staff. Work Hours: Full-Time (40 hours/week), typically Monday - Friday from 7:30 AM - 4:00 PM (including 30 min lunch). Evenings, weekends and/or holiday shifts may be required. Intermittent phone coverage on nights and weekends may be required. Shift assignments are subject to change depending on the operational needs of the department Desirable Qualifications Demonstrated commitment to caring for vulnerable and underserved populations including those with serious mental illness and substance use disorders Experience working with patients in a carceral setting Experience leading quality improvement and/or population health efforts in a clinical setting Experience providing care for complex patients with various socioeconomic, medical, and behavioral challenges At least one year of independent, full-scope adult primary care practice (Internal Medicine or Family Practice) including acute and chronic disease management How to qualify ️Required Before Applying️ Possession of a valid license to practice medicine issued by the Medical Board of California or the Osteopathic Medical Board of California AND Possession of valid Board Certification in the appropriate medical specialty area for the assigned facility or division. For some positions, possession of valid Drug Enforcement Agency registration with the United States Department of Justice is a special condition that is required in addition to the standard minimum qualifications associated with this job classification.
