Performs School Health services by providing guidance, support and directions to ensure consistent provision of quality patient care in consideration of the diverse physical, emotional, spiritual and socio-economic needs and beliefs of families and the community in accordance with all Tribal, state, federal and licensing standards and guidelines; Performs assessment of complex patient care information from abstracted data contained in medical records using specific criteria to determine severity of identified deficiencies Essential Functions: Serves as patient advocate by assisting patients with information needed to make their health care decisions, implements health care plans and seeks medical assistance. Maintains detailed records to measure effectiveness of program and suggest ways to improve throughout the process. Reviews clinical progress of patients and coordinates communications to referring, consulting, and primary care physicians. Plan, organize and provide direct care to the patient, as appropriate, if called upon to do so. Schedules follow-up appointments and provide information and reminders to patients & physician offices. Tracks outcomes of referrals and treatments Participates in mandatory in-services related to CPR, Infection Control; Blood Borne Pathogens, ATP proficiency etc.; Participates in CE programs containing moral, ethical and legal issues. Uses nursing processes to determine delivery of nursing services in accordance with departmental practices. Observes and accesses patient condition with consideration for age specific and discusses significant changes with primary care physician. Records initial assessment data and assessment data on the automated Electronic Health Record system Prioritizes care needs to presenting complaint, follows HP/DP guidelines and standards for specific groups, such as diabetes, hypertensive and other chronic groups. Facilitates continuity by involving the patient and family in the development of the plan for ongoing care. Performs patient and family education based on established standards as required by age of patient, and documents education in Electronic Health Records. Recognizes the dignity or worth of individuals, respects cultural, spiritual and psychosocial differences and apply philosophical and ethical concepts that promotes equality and continuity of care. Follows established procedures/guidelines for maintenance of storage of supplies and equipment.
