A small hospital in Western Virginia just outside of Roanoke is looking for an RN Case Manager to join their team. In operation since 1972, this is the only hospital located in the County and provides care to a primary service area of approximately 36,000 individuals including Wythe and Bland counties. We. employ over 350 full-time personnel and is staffed by over 80 physicians. The hospital operates under the guidance of a Board of Directors. We are searching for a RN Case Manager
Job Summary
Under the direction of the Case Manager director or another supervisor, assumes the responsibility and accountability for developing, evaluating and directing the plan of care for assigned patients. Works in collaboration with all other care disciplines to provide safe, efficient, and coordinated care for assigned patients. Responsible for facilitating appropriate health care services throughout the continuum of care. Uses collaborative and systematic processes to assess, plan and coordinate the implementation of services that meet the individual's health needs.
This position will be required to work every fourth weekend.
Qualifications
- Current licensure in the Commonwealth of Virginia as a Registered Nurse, and in good standing with the Board
- Bachelor of Science in Nursing preferred.
- Case Management experienced preferred
- Knowledge of utilization management and reimbursement mechanisms preferred.
