Responsibilities will include: Initial client assessments to determine care needs Development of individualized client care plans Case management including building and maintaining relationships with assigned clients Supervisory field staff visits Perform re-assessments and care plan updates as needed to maintain client's optimal status Caregiver orientation to start of case Provide in-person and on-call support to clients and caregivers as needed Maintain client records Caregiver competency assessments Requirements: Valid NJ RN license 1 year supervisory experience preferred 1 year home care experience preferred
