Employment Type: Part time Shift: 12 Hour Evening Shift Description: 0.6FTE 11a-11p with prn weekend commitment The Registered Nurse (RN) is responsible for assessing, planning, implementing and evaluating the delivery of patient care. The professional nurse assumes the responsibility and accountability for the delegation of patient care to other members of the healthcare team. Participates constructively in PI activities in the department. Can discuss performance improvement activities for the department. Contributes ideas for cost reductions, improved efficiency and outcomes without sacrificing quality. Maintains awareness of cost containment and waste reduction practices. Maintains and uses material, supplies and equipment in a cost-effective manner. Encourages and contributes innovative ideas and ways of doing things that increase efficiencies by challenging oneself and others to not accept the status quo. Recommends changes to policies, procedures and environment to enhance everyone's ability to provide optimum service to all patients, visitors and employees. Demonstrates understanding of PDSA (plan, do, study, act.) process and Just Culture. Keeps abreast of regulatory requirements for assigned unit. Recognizes special needs/concerns of populations served and adjusts communication accordingly, i.e., language barrier, diminished hearing, concrete terms, etc. Creates a safe and therapeutic environment that supports the special needs of the population served. Demonstrates effective abuse free verbal and non-verbal communication with residents/patients, their families, visitors and staff, i.e., communicates in a respectful manner of the person to whom communicating stage of development and cultural background. Organizational and Departmental Policies and Procedures Actively participates in staff meetings and/or committees on a regular basis. Aware of access to policies on the MHS intranet, HRHQ (i.e., Administrative Manual, HR policies, etc.) Seeks information and clarity when unsure of policy prior to initiating procedure. Reports any signs of impairment, suspicious behavior or diversion of drugs to supervisor promptly. Ensures that all equipment is in proper working order, and takes malfunctioning equipment out of service. Reports malfunctions promptly. Maintains a safe, neat clutter free work environment. Assumes ownership of cleanliness and neatness of work area and throughout the organization. Reports errors, environmental hazards and injuries to supervisor promptly. Nursing Process/Plan of Care Provides total, comprehensive care to patients by applying the nursing process in an organized and systemic manner according to hospital policy. Communicates proactively to the health care team, plan of care, changes in patient condition, the patient progress, the discharge plan and other relevant information. Completes admission according to policy utilizing a multi-system approach for the assessment and reassessment. Documents assessment/reassessment, plan, interventions and outcomes according to policy and interprets and utilizes clinical data in the plan of care: Documents pain assessment and plan of care. Documents fall risk and plan of care Documents wound risk and plan of care. Documents suicide assessment and plan of care. Documents infection control assessment (MDRO protocols). Performs accurate and thorough nursing physical assessment and documents thoroughly. Reassesses patient condition per policy. Interprets & utilizes clinical data in the plan of care. Assesses patient for potential risk factors. Recognizes and integrates patient special needs and individualizes care. Includes pain management and supportive care issues in plan of care. Adheres to infection control standards specific to patient care. Assumes accountability for managing the delivery of patient care. Responsible for learning policies and nursing procedure and communicates to others. Performs accurate and thorough nursing physical assessment. Participates in discharge planning, using a multi-disciplinary approach. Participates in multidisciplinary rounds. Assists patient/family with grieving process and end of life cases as appropriate. Well-Coordinated Patient Care Prioritizes and organizes work assignment and adjusts priorities based on changing situations. Completes work within a predetermined time frame. Acts as a patient advocate and upholds patient's rights. Promotes and maintains a safe, clean and orderly environment. Consults and refers to other Health Care professionals. Refers ethical concerns to appropriate resources. Delegates work to the care team as per State Board, job description and skill level Interacts professionally and communicates effectively with patients, visitors, physicians and co-workers. Plans delivery of care according to medical plan, patient acuity and staff competencies Anticipates potential length of stay or discharge problems. Is knowledgeable of Admission/Discharge criteria for the unit; and assists with timely patient bed assignment and discharge. Cooperates with all areas in the admission and transfer of patients throughout the hospital to ensure efficiency. Assists in promoting and enforcing the coordination of patient care. Keeps physicians informed of patient condition. Receives patients from other units in a timely manner. Verbal orders and "Read backs" are performed according to policy. Patient Education Assesses learning needs, readiness to learn, and actual or potential barriers to learning. Integrates primary and secondary diagnosis into the educational plan. Incorporates education into patient activity, (i.e., medication, equipment, food-drug interactions). Individualizes educational plan based on age, cultural and spiritual values. Ensures that appropriate patient education is provided to meet individual/ family needs. Documents patient education in process plans section in EHR. Assessment of Personnel Plans and coordinates patient assignments. Assists in the coordination of staff on a daily basis to assure delivery of patient care by competent caregivers. Communicates on an ongoing basis the staffing patterns to the Patient Care Manager. Promotes supervision and guidance of nursing personnel, including pool and agency staff. Provides a climate conducive to educational experiences for nursing students and new employees. Functions as a preceptor when necessary. Recognizes roles of non-RN staff based on Hospital policy and State Board regulations. Provides a climate respectful of cultural diversity. Emergency Response Maintains BCLS/CPR and specialty training (ACLS, PALS) as required. Recognizes changes in patient condition & responds appropriately. Recognizes and initiates "Stroke Protocols" as indicated. Recognizes and initiates "Rapid Response" as needed per policy. Performs in RN role within the resuscitation team. Reprioritizes and delegates other responsibilities during emergency situation. Functions calmly and efficiently in an emergency situation so that the patient's immediate needs are addressed and met. Proficient in the use of emergency equipment: Properly uses Bag-Valve-Mask resuscitator. Locates commonly used emergency items and medications on the crash cart. Demonstrates proper placement of defibrillator paddles. Checks and tests crash cart/defibrillator, according to policy and documents Keeps family/significant others informed of patient status. Demonstrates proficiency in end of life policies (i.e.; DNR, (Organ Donation) Gift of Life). Documents emergency interventions according to policy. Cares for and maintains dignity of deceased patients (post mortem, medical examiner/autopsy. Safety Utilizes safety features with sharps system, as per manufacturer's guidelines. Initiates and maintains isolation precautions appropriately (including notification of Infection Control Coordinator). Follows policy guidelines for prevention of urinary tract infections Demonstrates patient transfer techniques. Demonstrates ability to prevent, recognize and report abuse. Demonstrates ability to prevent, recognize and report infant/child abductions. Demonstrates awareness of Safe Haven Protocol. Demonstrates appropriate use of an alternative to restraints including least restrictive measures and documents appropriately. Demonstrates essential skills and knowledge to assist with patient on suicide precautions - 1:1: or close observation. Assures patient is never left alone. Assures staff remains within arms length. Monitors patient for potential risk factors and complications Demonstrates Patient/Resident Injury Prevention Measures. Demonstrates knowledge of procedure for handling an elopement. Recognizes fall risk associated with the patient population and implements fall reduction strategies. Manages the care of the agitated patient. Demonstrates knowledge of Universal Protocol (Time out) procedures and documents per policy. Demonstrates knowledge and ensures usage of hand-off communication forms (report form, Ticket to ride, pre-procedure checklist, etc.) and completes consistently. Maintains radiation safety precautions. Administers and documents blood products according to policy (consent, identification, adverse reactions, transfusion record, patient/family, education).
