Part-time 208 Mill Road Nursing/Nursing Mgt Day shift Work for One of the World's Best Health Systems. Hours: 32hrs Shift: Day shift Location: Diabetes Management - Fairhaven, MA; This position involves seeing patients at multiple session sites Travel: Driving on average 3 or more times per week required We are searching for a Registered Nurse or Registered Dietician certified in CDCES to provide diabetes self-management education/training (DSMT) and/or medical nutrition therapy (MNT) to individuals, family members and groups. Manages the care of a distinct patient population through assessment of educational needs, nutritional guidelines, lifestyle interventional goals and counseling. Coordinates patient care in collaboration with other healthcare providers (physicians, pharmacists, case managers, social workers, etc) to develop effective individualized protocols (medications, therapy, nutrition, devices, etc.) that empower patients with the techniques and behavior modification to help control their disease, avoid potential complications and bring about lasting improvements in their quality of life. May provide functional or technical supervision over other employees in same or closely related classifications where scope of supervisory responsibility is limited to occasional direction, guidance, explanation, etc. Education and/or Experience Requirements: Bachelor's Degree in Nursing is preferred. Over one year of related experience required; over three years preferred. Must have Certified Diabetes Care and Education Specialist credential (CDCES) Must be fully vaccinated against seasonal Influenza and the COVID 19 virus or to be exempt from the requirement for medical or personal reasons by signing a statement certifying you are choosing to be exempt from vaccination once hired.
