Responsibilities of an MDS Coordinator :
- Conduct and coordinate the Minimum Data Set (MDS) assessments accurately and timely for all residents.
- Collaborate with interdisciplinary teams to gather information and develop comprehensive care plans.
- Ensure compliance with federal and state regulations related to MDS assessments and care planning.
- Participate in the development and implementation of policies and procedures related to resident assessments.
- Act as a liaison between the nursing staff, physicians, and other healthcare professionals to communicate resident needs and changes in condition.
Qualifications of an MDS Coordinator :
- LPN/RN licensed in PA
- Responsible, ready, and eager to work
