TITLE: Assisted Living RN/LPN EXEMPT STATUS: Non-Exempt REPORTS TO: Memory Care Nursing Supervisor/SALSA GENERAL SUMMARY: The Staff Nurse is responsible for the accurate and timely administration of medications, treatments, documentation and emergency calls as indicated for The Cottage at Avalon. We pay attention to details Essential Job Duties: Staff Nurse may assist the Registered nurse with the following which shall be documented in the clients service record Admissions, development of the client service program and instructions for assisted living aide services. Assessments, completed as often as necessary asked o the client's condition but not less frequently than every one hundred and twenty days, and prompt actions when a change in the client's condition would require a change in the clients service program Coordination of services with the client, family and other appropriate individuals involved in the client service program Participation in the orientation, teaching and supervision of Resident Aides Arrangements for training or supervision of the Residents Aides by other professionals when appropriate. Referral to appropriate professionals or agencies whenever the clients condition necessitates, including the provision of current clinical information ensuring that if the clients condition is no longer chronic and stable services of a licensed home health care agency are engaged or other appropriate arrangements are made. Planning for clients who shall no longer receive or require the services of the assisted living services agency. Implementation or delegation of responsibility for the availability of nursing services on a twenty four (24) hour basis Nursing services which shall include but not necessarily be limited to Client teaching Wellness counseling Health promotion Disease prevention Medication administration and delegation of supervision of self-administered medications as specified in subdivision of this subsection Provision of care and services to clients whose condition are chronic and stable Other Duties: Assists in admission and discharge procedures. Responds to resident emergency call system as indicated. Makes rounds with physicians and transcribes orders. Keeps physician and family aware of change in resident's condition. Maintains the self respect, personal dignity, and physical safety of each patient. Assures continuity of services and care. Attends in-service training and education sessions, as assigne Performs specific work duties and responsibilities assigned by supervisor. QUALIFICATIONS: RN or LPN currently licensed by the State of Connecticut, current CPR certification required At least 6 months nursing experience. Geriatric involvement preferable. PHYSICAL REQUIREMENTS/WORKING CONDITIONS: Frequent standing, stooping, bending, stretching, squatting; must be able to stand/walk for up to 70% of the work day; must be able to lift , transfer, turn and position residents weighing up to 125 lbs. unassisted, or with the assistance of one for a resident weighing over 125 lbs. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently and 50 lbs. occasionally. May be exposed to communicable diseases including the HIV and HBV viruses. Must be able to work paying close attention to detail with frequent interruptions.
