SA0859 - RN Home Health - Rockford, IL - 8 Weeks Contract Sigma Healthcare team is looking for experienced Home Health Registered Nurse to work with our client on one of their teams.
Shift: 08:00 hr Days( 8a-5p M-F, every 3rd weekend., rotating holidays)
Home Health RN: - Responsible for traveling to patient's homes or skilled nursing facilities in a localized area around the regional office to assess and perform skilled nursing services. As well as document assessments, actions and outcomes.
Responsibilities: - Evaluate patients and develop appropriate care plans
- Apply care plans according to individual patients
- Assess outcomes and manage care plan modifications as needed (documentation through OASIS software)
- Provide nursing services based on the care plan devised (G-tubes, J-tubes, Trachs, wound care, etc.)
- Supervise nursing assistants in their everyday work such as bathing and grooming
- Administer medication as instructed
- Manage emergency procedures
Requirements: - Current and valid RN license.
- Currently BLS certified
- At least one-year experience as a professional nurse within the past three years in an acute care setting.
- Experience in home health, IV therapy, ER or oncology preferred.
- Cardiac or wound certification preferred.
- Covid vaccine required, including booster for those eligible (5 months after last dose).
- Medical/Religious exemptions must be tested weekly and be prepared to provide proof should it be requested.
