Position Highlights:
- Work location: Home base of 1920 Libal St, Green Bay, WI 54301. Position travels to patient's homes within the region (we offer mileage reimbursement and/or use of company vehicle).
- Conduct in-home visits to complete a comprehensive physical assessment and provide care such as IV medication administration, IV site maintenance, wound care/wound vac application, and Foley/ostomy care.
- We serve patients in portions of 9 counties in Northeast Wisconsin.
- Salary is based on experience and will be provided before interview.
Shift Hours:
Part-Time availability between 0.5-0.7FTE (20-28 hours/week), Hours 08-1630 during the work week, and 08-1630 on the weekends/holidays. Call requirements generally 1x/week, and on scheduled weekends. Nurses work every 4thweekend.

Nurses use advanced clinical skills to provide skilled, short-term acute care services, engage with one patient at a time, and build trusted patient relationships. Travel to patient homes to conduct in home assessments and provide hands on care such as administration of IV medications, provide wound, Foley, and ostomy care.
Provide patient education and coaching to make a meaningful impact and improve patient outcomes. Work collaboratively with the multi-disciplinary health care team and community resources to provide the best outcomes for patients.

Qualifications:
Wisconsin RN license required.
Valid Wisconsin driver's license in good standing and liability car insurance required.
