Job Description: The Educator, Case Management & Social Services, reports to the Director, Case Management. This key leadership position has responsibilities with both clinical professional development activities as well as activities to promote and enhance quality patient outcomes. This role intertwines responsibilities for process improvement, patient outcomes, clinical education, program management, leadership, staff coaching/mentoring and case management research/evidence-based practice in an integrated, cross-campus delivery.

Experience:
3 years Nursing or Case Management / Social Services Required
Acute Care setting Required
Experience in Education/Training - Education Preferred
Case Management Required
Experience with Quality metrics and action planning Preferred
Interqual Required
Certifications/Licensures:
RN Registered Nursing - California Board of Nursing Required or
LCSW Licensed Clinical Social Worker - California Board of Behavioral Sciences Required and
BLS Basic Life Support - American Heart Association Required
ACM Accredited Case Manager - ACMA American Case Management Association or
CCM Certified Case Manager - CCMC Commission for Case Manager Certification Preferred
Work Shift: 08.0 - 08:00 - 16:30 No Waive (United States of America)
Pay Range:
$80.34 - $109.47 Hourly
Offer amounts are based on demonstrated/relevant experience and/or licensure.
