Job description
Our Client is seeking an RN Clinical Supervisor to provide oversight of
all patient care services and personnel. Oversight must include the following: is
responsible to makes patient and personnel assignments, coordinates patient
care,
coordinates referrals, assures that patient needs are continually assessed and
assured the development, implementation and updates individualized plan of care.
Also, is a Professional Staff who is accountable for maintaining the quality of
patient care in accordance with the agencies objectives and policies, through
planning,
coordination, implementation and evaluating the home health services provided.
RN Clinical Supervisor starting pay: $120,000.00 per year
RN Clinical Supervisor Responsibilities:
Coordinates and schedules the daily workload of each agency personnel to
their fullest capacity.
Provides daily contact supervision of field nurses, other healthcare
professionals, and home health aides to assure the quality of services and
maintenance of standards.
Reviews and investigates the clinical content, diagnosis, medical history,
medications, fee status and other procedures of the record of patients
under their supervision.
Directs staff toward the appropriate use of community resources.
Appraises performance at probation and annually with the Director of
Nursing. Provides the teaching and counseling necessary to enhance the
delivery of services and Professional development of nursing, professional
and health aide staff.
Identifies and evaluates nursing needs of the patient and appropriateness
of home care by review of the initial intake screening form; prior to
assignment of direct care staff.
Represents and participates in community related functions as necessary.
Assists with the orientation of new professional staff to agency policies and
procedures.
Sees patients in the home setting as needed.
Takes on-call as directed.
Participates in audits, and interdisciplinary team conferences and represents
agency in the community as requested.
Submits to the Director of Nursing yearly self-evaluations, goals and
objectives.
Reviews weekly schedule done by nurse and other staff for changes or
updates.
Reminds each nurse to check Medicare, or any other program eligibility
information every month. Notifies Board of Directors, DON, and QAPI staff
of any changes in status immediately.
Fills out the patient's re-hospitalization referral/admission form sheet daily
and gives to intake department with recertification date written. Review
OASIS assessments, sign-up package weekly.
Does monthly management summary reports. Gives to Director of Nursing.
Ensure HIPAA guidelines and procedures are maintained.
RN Clinical Supervisor Qualifications:
Licensed Registered Nurse in the State of California.
Must be carry a valid, verified licensed in the State of California.
Knowledgeable of Insurance, Medicare and Medicaid guidelines.
Good communication skills and ability to establish good rapport with other
staff members.
Working knowledge of home health care and the principles, ethic and
techniques of professional nursing and required documentation thereof.
Skillful in organization of work and in principles of time management; and
knowledge of management process.
One or more years in home health agency. At least one year managerial
responsibilities.
Able to concentrate on detail with frequent interruptions.
Able to comprehend and utilize professional education materials.
