RN Clinical Care Coordinator

We are in search of a RN Clinical Care Coordinator to supervise, provide and coordinate nursing care in compliance with facility and regulatory policies and procedures and to assess residents' care needs.

Primary functions:
Interview residents, residents' representatives and others with information related to the Minimum Data Set (MDS) assessment.
Collect all data necessary to complete MDS forms and related documentation.
Make appropriate referrals to other health care professionals.
Participate and/or conduct interdisciplinary meetings.
Supervise and participate in the overall care of the residents
Oversee clinical coordination, review labs, assist the physician with rounds
Participate in the facilities Quality Assurance process to review facility systems as directed

Wage Information:
$5,000 SIGN ON BONUS
$37.15/hour -$41.65/hour based on experience ad an RN & LPN
Including $0.85/hour MI pass through pay until 09.30.24
