Virtus Healthcare Staffing is partnering with a leading Level I Trauma Children's Hospital in Philadelphia, PA to find a dedicated RN Case Manager - Special Needs to join their exceptional team.

About the Opportunity:

This is a fulfilling chance for a registered nurse to play a vital role in coordinating care and advocating for children with special needs within a dynamic and supportive environment. You will work collaboratively with a team of healthcare professionals to ensure these children receive the comprehensive care and resources they deserve.

Responsibilities:

Conduct comprehensive assessments to identify the unique needs of each child with special needs.

Develop and implement individualized care plans in collaboration with patients, families, physicians, and other healthcare providers.

Coordinate and manage care across various departments, including inpatient, outpatient, and community-based services.

Educate and empower patients, families, and caregivers on managing special needs.

Advocate for children with special needs and ensure they have access to necessary resources and support systems.

Monitor and track patient progress, documenting care interventions and outcomes.

Maintain accurate and detailed case management records.

Participate in quality improvement initiatives to enhance the care provided to children with special needs.

Qualifications:

Active and unrestricted Pennsylvania Registered Nurse (RN) license.

Minimum 2 years of experience in pediatrics or a related field, with a focus on special needs preferred.

Strong understanding of special needs conditions and best practices in care management.

Excellent communication, collaboration, and interpersonal skills.

Compassionate and patient-centered approach to care.

Ability to work independently and as part of a team.

Exceptional organizational and time management skills.

Proficient in electronic health record (EHR) systems.
