Bartlett Regional Hospital is searching for candidates interested in a dynamic nursing opportunity in our Emergency Department. Case Management is a department that supports all areas of the hospital, including the Emergency Department. Case Management is comprised of nurses and social workers that work together to provide coordinated care for our patients.

In the Emergency Department, the nurse case manager works closely with the social worker as they provide support to patients and families in the ED as well as the bedside team when needed. The case managers identify what is needed in the moment for coordination of care, as well as the bigger picture as they work to support long-term well being.

This role is in a fast paced environment and is responsive to multiple teams and partners. Beyond the direct patient care, the case managers are involved with identifying programs, services, and projects to support patient and community needs. They participate with community coalitions to collaborate and support coordination of care and improvement in services.

What You Will Do:

  • Support course of care and/or discharges from the ED in real-time including, but not limited to: multi-visit patients, patients with substance misuse, poor social situations, tourists, patients/families experiencing trauma, death, grief.
  • Post-discharge call backs for ED patients to ensure patients understand their discharge plan and for further coordination as indicated.
  • Collaborate with the ED SW Case Managers and treatment team on EDie care guidelines, reducing re-admissions for multi-visit patients, tracking outcomes, running reports, follow-up with community partners.

Minimum Qualifications for the Job:

Education: Graduation from an accredited School of Nursing. Bachelor's of Science in Nursing preferred.

Experience: Three (3) years of acute care nursing experience.

Preferred Experience: One (1) year experience in utilization review, case management, care coordination, quality management or similar role is preferred.

License(s): Current State of Alaska Registered Nurse license. (must meet education requirement(s) for Alaska State licensure)

Certification(s): TEAM and BLS within three (3) months of appointment.

Internal Hires: Pay rate will be determined based on applicable personnel rule or union contract terms.

Originally known as St. Ann s Hospital, the hospital opened in 1886 and was operated by the Sisters of St. Ann until the 1960s. In 1965, the hospital s operations were turned over to the City and Borough of Juneau, and a new facility was built at the hospital s present location on Hospital Drive.

Bartlett Regional Hospital is licensed for a total of 57 inpatient beds and 16 residential substance-abuse treatment facility beds in the Rainforest Recovery Center. The hospital serves a 15,000-square-mile region in the northern part of Southeast Alaska. Approximately 55,000 people reside in our service area, with most communities inaccessible by road.
