RN-Case Manager

Job Description

Monitors and manages patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance.

Reports to: Director Quality Services

Minimum Education
Associate Degree, Bachelor Degree or RN Diploma required. Drag Edit Delete Required Skills
Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. Drag Edit Delete Required Skills
American Heart Association or American Red Cross Certifications: Basic Life Support (BLS) required upon hire or must be obtained within 90 days of employment Drag Edit Delete Required Skills
License: Current TN or Multi-State RN licensure required Drag Edit Delete Minimum Work Experience
One to two years experience working in a hospital (acute or critical access) setting or case management experience in hospital setting (acute or critical access).
